Before submitting an application:
CITI ethics training

(register, select FHSU, “Social and Behavioral Research-Basic/Refresher Course”)
Lasts 3 years
Submit an IRB application
To use IRBNet: register, then affiliate w/ FHSU (just like CITI training site)
Usually need to submit at least 4 documents:
Application
Informed consent
Data collection instrument (survey/interview/focus group questions)
Recruitment flier/email/announcement
IRB only cares about possible risk to subjects (but may provide suggestions beyond that)
Possible categories: depend on
minimal risk
Not research (program improvement, not generalizable knowledge)
Exempt (for us: education assessment, minimal-risk deidentified data collection)
Limited (secondary research with identifiable records): IRB admin & IRB chair
Expedited (minimal-risk research): IRB admin & 2 reviewers
Full (more than minimal risk)
